Please use this form to register your team for the YPQA Coaching workshop with Weikart Center Trainer Bob McNeil. Please list any food accommodations  that anyone on your team will require in the space provided.

When: Refreshments at 8:30, training from 9 until 4 p.m. on December 2

Where: Goodwill Industries located at 4805 North 72 Street .

All attendees must be registered no later than November 28th. Registration is required for attendance. 

*Agencies will be billed directly for food and supplies for those who  register but do not attend the training.
